AI pathology · MR enterography

Three modalities. One read.

PAIR (Perspectum AI Reader) is the platform family for AI-supported quantitative central reading in pharma trials. PAIR-MASH™ is the live product anchoring the family; PAIR-IBD and other indication SKUs follow. PAIR is built to triangulate AI pathology, MR enterography, and digital endoscopy in a single audit-ready central read: pathology and MRI today, with digital endoscopy on the roadmap. Where Path.AI AIM reads pathology alone, PAIR brings the proven imaging modalities through the same vendor that already runs your imaging biomarkers.

What PAIR sees, across three modalities.

Pathology and MR enterography, quantified together and reconciled in one report as a single endpoint, with digital endoscopy on the roadmap.

Modality 1 · AI-pathology

Quantified gut histology.

Crypt architecture, neutrophil infiltration, ulceration extent, chronic inflammation. Mayo, Geboes, UCEIS, and an anatomy-anchored scoring stack from Deshpande et al. (J Clin Pathology, 2025). Each call comes with a confidence score and a heat-mapped region.

Modality 2 · Digital endoscopy (roadmap)

Mucosal disease, scored.

On the roadmap: endoscopic video and stills processed through the same auditable pipeline, with Mayo Endoscopic Subscore and UCEIS scoring, the same chain-of-custody and regulator-ready output. Especially relevant for ulcerative colitis and pouchitis.

Modality 3 · MR enterography

Transmural disease, mapped.

Quantitative MRE: bowel wall thickness, T1/T2 inflammation maps, perfusion, fistula and stricture tracking. The Crohn’s-specific gap that biopsy AI alone cannot close. Built on Perspectum’s 14-year quantitative MRI heritage.

Three modalities. One read. Path.AI does one. We do three.

Sponsors running large IBD trials want one set of numbers to defend to the IDMC and the regulator. PAIR gives them that, not three datasets stitched together post-hoc, but one auditable triangulated endpoint.

Evidence & regulatory

Built on the same evidence engine that produced cT1.

PAIR is the newest expression of Perspectum's quantitative-imaging programme, same Oxford pedigree, same peer-reviewed approach to evidence generation. Live in active sponsor evaluations, with the IBD white paper published.

Three modalities, one read.

Most IBD trial endpoints come from one modality. PAIR is built to bring the three that decide IBD, pathology, endoscopy, and MR enterography, into a single auditable report, with digital endoscopy on the roadmap.

Modality 01

Quantitative pathology

AI-scored biopsy histology. Mayo, Geboes, UCEIS plus the anatomy-anchored Perspectum scoring stack. Target slide-to-report turnaround of days, not weeks.

PAIR-IBD core
Modality 02

Digital endoscopy

Designed for AI-scored endoscopy, stills and full video, anatomy-anchored. UCEIS and Mayo endoscopic subscore, with full chain-of-custody for central read.

On the roadmap
Modality 03

MR enterography

Quantitative bowel wall thickness, T1/T2 inflammation maps, perfusion, fistula tracking. The Crohn's-specific signal that biopsy AI cannot reach.

The differentiator

One platform. A family of named SKUs.

PAIR-MASH™ is the first product in the family. The family extends with PAIR-IBD and its indication-specific SKUs as each next indication’s evidence stack supports its own launch.

Live

PAIR-MASH™

MASH histology · Pathology-led, integrated with LiverMultiScan

Quantitative AI assessment of ballooning, fibrosis, steatosis, lobular, portal, and interface inflammation. Trademark filed. Live with 10+ Phase 3 MASH trials and 20+ pathology publications behind it.

PAIR-UC

Ulcerative colitis · Endoscopy-led

Mayo, UCEIS, Geboes. Largest IBD trial market today. Sequencing follows the AIM-HI UC competitive opening.

PAIR-IBD family · roadmap

PAIR-CD

Crohn’s disease · MRE-led

Transmural disease. Where the imaging moat is most defensible.

2027 · Roadmap

PAIR-Peds-IBD

Pediatric IBD · Imaging-first

Imaging-first by clinical mandate; biopsy is harder in children.

2027 · Roadmap

PAIR-Perianal

Perianal Crohn’s · MRE-led

Fistulating disease. Big unmet need in surgical and medical care.

2027 · Roadmap

PAIR-Pouchitis

Post-colectomy · Endoscopy-led

Underserved by existing AI vendors. Adjacent to UC follow-up.

PAIR family name and PAIR-MASH product mark filed for trademark; per-indication SKU names for PAIR-IBD and its sub-SKUs finalise as each indication launches.

Working draft, pre-empts the scientific positioning the team will formalise. Treat as a starting point, not the deck.

Built for trials first, clinical follows the evidence.

PAIR is in MASH and IBD programmes with sponsors today. It reaches clinical care behind the trial data, one indication at a time.

Designing a MASH or IBD programme? Let’s talk early.

PAIR integrates cleanest when it’s in the protocol from the start. PAIR-MASH is the current live product in the family. We’ll come back with a programme-specific brief within five working days.
